#6-10E. Overriding Patient Wishes: Dementia, Advance Directives, and Surrogate Decision-Making (JAMA Netw Open, 2026)
2026年10月29日 13:00·5分25秒
How reliably do advance directives guide surrogate decisions when patients develop dementia? A national survey study of 5,965 US adults revealed that participants were 18.6 percentage points less likely to recommend life-sustaining treatments for critically ill patients with dementia than for those without, even when explicit advance directives requested all possible care. While relying on hypothetical vignettes rather than real-time bedside decisions limits emotional context, these findings emphasize that written directives alone cannot guarantee preference-concordant care. Clinicians must actively facilitate ongoing goal-of-care discussions with patients and surrogates to align end-of-life decisions with true patient values.
