#1-99E. Unveiling Lymphoma Predictors in Primary Sjögren Disease (Arthritis & Rheumatology, 2025)

How can we more accurately identify patients with primary Sjögren disease (pSD) at the highest risk of developing non-Hodgkin lymphoma (NHL)? In a recent study published in Arthritis & Rheumatology, researchers conducted deep immunophenotyping on 206 pSD patients to uncover novel predictive biomarkers1. The study's most critical finding is the strong association of two specific cell populations—CD11c+ FcRL5+ tissue-like memory B cells and IFNγ+ TNFα+ conventional T cells—with NHL in pSD2. Impressively, the combination of these two biomarkers alone demonstrated a sensitivity of 78.9% and specificity of 76.8% for identifying NHL patients, outperforming the sum of conventional clinical and biological markers2. This discovery could be pivotal for stratifying high-risk patients who might otherwise be overlooked, enabling earlier intervention3. However, it is important to note that this was a cross-sectional study with a small number of NHL cases (n=19), warranting further validation in larger, prospective cohorts4. For clinicians, these findings provide a promising new avenue for risk assessment and could significantly refine patient management in pSD56.
