#1-93E. Latest Guidelines for FMF Management: EULAR/PReS 2024 Recommendations (Annals of the Rheumatic Diseases, 2025)

In managing Familial Mediterranean Fever (FMF), is controlling acute attacks enough, or should we aim for complete control of subclinical inflammation? This paper presents the 2024 updated EULAR/PReS recommendations, providing a new framework with 4 overarching principles and 12 recommendations for FMF treatment and monitoring based on the latest evidence. The core finding is the high-level evidence supporting IL-1-targeting treatments as an additional therapy for colchicine-resistant patients, which clarifies the next therapeutic step for these challenging cases. However, a critical point to consider is that a standardized definition for "minimal disease activity" to gauge treatment response is still lacking, which poses a challenge for clinical application.
