#4-20E. Uncoupling Thrombosis from Hemostasis: Asundexian for Secondary Stroke Prevention (NEJM, 2026)

Are you struggling to balance the risk of recurrent ischemic stroke against the increased danger of major bleeding when intensifying antithrombotic therapy? In the phase 3 OCEANIC-STROKE trial, patients with noncardioembolic ischemic stroke or high-risk transient ischemic attack (TIA) received the factor XIa inhibitor asundexian in addition to standard antiplatelet therapy. The addition of asundexian significantly reduced the risk of recurrent ischemic stroke compared to placebo without increasing the rate of major bleeding events. The most critical clinical takeaway from this study is that inhibiting factor XI successfully uncouples pathologic thrombosis from normal hemostasis, effectively preventing recurrent strokes without elevating bleeding risks—a previously elusive goal in secondary prevention. However, when critically appraising this study, clinicians should note the limited representation of patients with severe strokes (an NIHSS score of 8 or higher) or high-risk TIAs, which may restrict the generalizability of the findings to these specific subgroups. For physicians, this research is highly relevant as it highlights a promising, safer therapeutic option for secondary stroke prevention, allowing us to better protect our patients from recurrence while minimizing the fear of severe hemorrhagic complications.
