#4-03E. Targeting the Untreatable: BCMA-Directed T-Cell Engagers in Autoimmune Diseases (NEJM, 2024)

How should we manage severe autoimmune diseases that remain highly resistant to conventional B-cell depletion therapies? In this episode, we explore a recent study examining the use of teclistamab, a B-cell maturation antigen (BCMA)-targeted T-cell engager, in four patients with multi-drug resistant autoimmune conditions, including systemic sclerosis, primary Sjögren’s syndrome, idiopathic inflammatory myositis, and rheumatoid arthritis. The study demonstrates that the administration of teclistamab successfully engaged T cells to deplete plasma cells, leading to a significant reduction in autoantibody levels and marked clinical improvements across various disease activity indices without severe toxicities. The most crucial clinical takeaway is that directly targeting long-lived plasma cells offers a promising new therapeutic avenue for refractory cases, highlighting a potential next-step intervention for patients failing standard treatments. However, when critically appraising this study, clinicians must note that the cohort size is exceedingly small with only four patients, and the long-term efficacy and safety of this approach remain completely unverified. Ultimately, this paper is highly relevant to our daily clinical practice because it introduces an innovative mechanistic strategy that could redefine our treatment algorithms for patients with otherwise untreatable, severe autoimmune diseases.
