#3-76E. Predicting Gout Flares: The Role of Metabolomics in Colchicine Prophylaxis (Arthritis Rheumatol, 2026)
2026年3月12日 17:00·4分57秒
Is it possible to individualize flare prophylaxis when initiating urate-lowering therapy (ULT) for patients with gout? This prospective cohort study investigated whether baseline serum metabolomics could serve as a reliable predictor of responsiveness to colchicine prophylaxis in patients commencing treat-to-target ULT. Researchers identified a specific metabolite risk score capable of independently predicting the risk of recurrent flares during the first six months of treatment. The most critical finding suggests that metabolomic profiles obtained prior to ULT initiation can effectively predict early gout flare risk, offering a novel biomarker for therapeutic outcomes. However, when critically appraising this study, clinicians must consider its methodological limitations, notably that the cohort consisted exclusively of male participants and heavily favored specific ULTs like febuxostat or benzbromarone, potentially limiting its broad generalizability. Despite these limitations, this research is highly relevant for daily practice because it provides a foundation for tailoring and optimizing the dosing and duration of colchicine prophylaxis on an individual basis, thereby enhancing clinical decision-making for patients initiating ULT.
