Are you struggling to manage flares and reduce steroid dependence in your patients with active systemic lupus erythematosus? In a recent phase 3, multicenter, double-blind, placebo-controlled trial, researchers evaluated the addition of obinutuzumab, a type II anti-CD20 monoclonal antibody, to standard therapy in adults with active SLE. At week 52, obinutuzumab demonstrated significant superiority over placebo in achieving the primary endpoint of an SRI-4 response. The most pivotal clinical finding is that obinutuzumab securely facilitates sustained glucocorticoid reduction and significantly prolongs the time to the first disease flare. However, when critically appraising this study, it is crucial to note that patients with severe active central nervous system lupus or recent proliferative or membranous lupus nephritis were excluded, and long-term safety beyond 52 weeks remains unestablished.
