#4-96E. A New Dawn for Lupus Skin Manifestations: The Enpatoran Phase 2 Trial (Lancet Rheumatol, 2026)

Are we finally entering a new era of targeted oral therapies for refractory cutaneous manifestations in lupus? In this episode, we review a phase 2 trial evaluating enpatoran, an oral Toll-like receptor (TLR) 7/8 inhibitor, in patients with cutaneous lupus erythematosus (CLE) or systemic lupus erythematosus (SLE) with active skin manifestations. The study demonstrated that enpatoran achieved a significant, dose-dependent improvement in the Cutaneous Lupus Erythematosus Disease Area and Severity Index-Activity (CLASI-A) score at week 16 compared to placebo. The most critical finding of this research is that it is the first to demonstrate that TLR7/8 inhibition rapidly improves clinical disease activity while effectively suppressing the type I interferon pathway. However, when critically appraising this trial, clinicians must carefully consider the relatively small sample size of approximately 25 patients per treatment group and the exploratory nature of several secondary endpoints. For practicing physicians, this study is highly relevant as it unveils a promising novel oral targeted therapy that could overcome the limitations of current standard treatments, which are often constrained by inadequate efficacy and considerable toxicity, ultimately offering a much-needed option for daily clinical practice.
