How do we optimize management for polymyalgia rheumatica patients who relapse or fail glucocorticoid tapering? In this retrospective comparative cohort study using US Medicare claims data, researchers compared the real-world effectiveness and safety of interleukin-6 receptor inhibitors (IL-6Ris) against conventional synthetic immunomodulators (csIM) in patients with steroid-refractory polymyalgia rheumatica. The key clinical finding reveals that patients initiating IL-6Ri therapy were significantly more likely to discontinue glucocorticoids or achieve minimal use (two milligrams or less per day) by year one compared to those on csIM, representing a crucial steroid-sparing benefit. However, when critically appraising these results, clinicians must consider the limitations of retrospective claims data, such as potential misclassification, coding bias for off-label reimbursement, and a higher risk of primary hospitalized infections during the first year of IL-6Ri use. Ultimately, this study provides pivotal first-of-its-kind real-world comparative evidence, supporting the clinical rationale for earlier biological intervention with IL-6 receptor inhibitors to mitigate long-term corticosteroid toxicity in steroid-dependent polymyalgia rheumatica.
