Can we redefine the treatment landscape for treatment-resistant Sjögren’s disease by identifying its underlying immune endotypes? In today's episode, we examine a pivotal study investigating the role of interferon-alpha (IFN-α) in driving specific Sjögren's disease endotypes. Utilizing ultrasensitive single-molecule ELISA and Olink proteomics on the UKPSSR and UK Biobank cohorts, researchers discovered that oligoprotein IFN signatures can be detected up to 14 years prior to a formal diagnosis. The most groundbreaking finding is the demonstration, via a novel transgenic mouse model, that chronic systemic elevation of IFN-α is a direct causal driver of a distinct immune endotype characterized by TRIM21 autoantibodies, rather than a mere epiphenomenon.
