#4-76E. Breaking the Glucocorticoid Trap: Secukinumab for Relapsed Polymyalgia Rheumatica (NEJM, 2026)

Are we finally breaking the cycle of glucocorticoid dependence in managing polymyalgia rheumatica (PMR)? In this episode, we explore a phase 3 randomized controlled trial investigating the efficacy of secukinumab, an IL-17A inhibitor, for patients with recently relapsed PMR. The study randomized 381 patients to receive either secukinumab or a placebo, alongside a 24-week glucocorticoid taper. The findings revealed that secukinumab significantly improved sustained remission rates at week 52 and reduced the annual cumulative glucocorticoid dose1more_horiz. The most critical clinical takeaway is that secukinumab effectively doubled the sustained remission rate to approximately 41% compared to 20% in the placebo group, strongly supporting its potential as a novel glucocorticoid-sparing agent.
