How well do our real-world cancer screening practices for patients with idiopathic inflammatory myopathies align with the recently established IMACS consensus guidelines? An international audit of 795 patients revealed a stark guideline-practice gap, with only 6.2% of high-risk patients receiving the full panel of recommended tests, though retrospective differential screening intensity limits the formal assessment of the framework's predictive validity. These findings urge clinicians to implement structured, risk-based screening strategies in routine care and recognize critical red flags, like smoking and night sweats, to improve patient outcomes.
