#5-47E. Targeting Ectopic Fat: Survodutide's Breakthrough in Obesity Management (NEJM, 2026)

Are you struggling to manage ectopic fat accumulation, such as liver fat, in your patients with obesity? This phase 3, double-blind, randomized, placebo-controlled trial evaluated once-weekly subcutaneous survodutide, a novel glucagon and GLP-1 receptor dual agonist, in adults with obesity without diabetes over 76 weeks. The most ground-breaking clinical finding is that the 6.0 mg dose of survodutide achieved a dramatic 63.1% reduction in liver fat and a 34.0% reduction in visceral fat, highlighting its unique potential to target ectopic fat deposition beyond standard weight loss. However, when critically appraising these results, we must note that gastrointestinal adverse events were highly prevalent, leading to treatment discontinuation in 20.2% of participants in the 6.0 mg group. This high discontinuation rate poses a real-world challenge for long-term clinical adherence. For clinicians, this study is crucial as it positions survodutide as a highly promising therapeutic option for obesity, particularly for those with metabolic dysfunction-associated steatohepatitis (MASH), where reducing visceral and hepatic fat is a critical therapeutic goal.
