#5-04E. Navigating Subsequent Pregnancies: APO Recurrence Risks in SLE (Rheumatology, 2026)

How does a complicated first pregnancy impact subsequent reproductive outcomes for women with systemic lupus erythematosus (SLE)? In this episode, we review a nationwide population-based cohort study investigating the rates of second deliveries and the recurrence of adverse pregnancy outcomes (APOs) among women with SLE. The study revealed that women with SLE are significantly less likely to have a second delivery, particularly if their first pregnancy was complicated, and they face a substantially higher risk of recurrent APOs compared to the general obstetric population. Most notably, the data demonstrate that the recurrence risk of preterm delivery is more than doubled (RR 2.31). This pivotal finding underscores that a history of APOs during a first pregnancy serves as a powerful predictive marker for future risks, emphasizing the critical need for individualized family planning and vigilant perinatal surveillance. However, when critically appraising these results, clinicians must consider the study's limitations, specifically the absence of data regarding pre-conception disease activity and early miscarriages, which could independently influence reproductive decisions and outcomes. Ultimately, these insights are essential for physicians, highlighting the necessity of integrated, proactive management and tailored reproductive counseling to optimize maternal and fetal health in clinical practice.
