How can we effectively manage uncontrolled gout when conventional therapies fail? This episode delves into the DISSOLVE I and II Phase 3 trials evaluating NASP (nanoencapsulated sirolimus plus pegadricase). The research demonstrates that monthly infusions of NASP significantly increase the complete serum urate response rate during weeks 21 to 24 compared to placebo. Crucially, targeted immunomodulation with NAS effectively mitigates the development of anti-drug antibodies, allowing for sustained urate lowering and significant tophus resolution.
